Remote Monitoring · Reimbursement

Billed separately from telehealth.

RPM and RTM are reimbursed under the Medicare Physician Fee Schedule — and, unlike many services, they stay categorized separately from telehealth. Here are the code families, the CY2026 final rule, and the reasons claims get denied.

CY2026 Physician Fee Schedule

What the CY2026 final rule changed

CMS evaluates new codes every year. The CY2026 PFS final rule — effective January 1, 2026 — delivered several long-awaited remote-monitoring changes:

Finalized

RPM & RTM under 16 days

New RPM and RTM codes now cover services with 2–15 days of data collection in a 30-day period — closing the long-standing 16-day gap, payable from January 2026.

Finalized

Expanded digital mental-health payment

Payment for the digital mental-health treatment (DMHT) device codes — G0552, G0553, and G0554 — now extends to FDA-cleared devices for ADHD therapy.

Still moving

RPM / RTM valuation

CMS collected feedback on how remote monitoring is valued — input now shaping the CY2027 proposed rule (July 2026), which would tighten who can furnish RPM/RTM and revisit device payment.

Digital tools for healthy lifestyles

CMS is considering separate coding and payment for digital tools that encourage healthy lifestyles as part of mental-health treatment plans.

Digital mental-health treatment codes

CMS pays for FDA-cleared digital mental-health treatment (DMHT) devices used alongside ongoing behavioral care through three HCPCS codes — proposed as GMBT1–3 and finalized as G0552–G0554, modeled in part on the RTM code family:

G0552

Supply of a DMHT device plus initial education and onboarding, per course of treatment, augmenting a behavioral-therapy plan.

G0553

First 20 minutes of monthly treatment-management services tied to the patient's therapeutic use of the DMHT device, requiring at least one interactive communication with the patient or caregiver that month.

G0554

Each additional 20 minutes of monthly treatment-management services for the DMHT device.

Why Claims Fail

Common reasons for reimbursement denial

Denial

Insufficient data collected

Data must be collected and transmitted for at least 16 days within a 30-day period.

Denial

Multiple billings per patient

Many of these codes can be billed only once per patient, per 30-day period.

Denial

Clinician ineligible for E/M

Like other E/M services, RPM services must be billed by a physician or a clinician practicing under supervision of a physician.

The fee schedule changes every year.
